Dear Parent/Guardian,
Nasal and Intramuscular Flu Vaccination Consent Form
Your child’s flu vaccination is now due
This vaccination is recommended to help protect your child against flu. Flu can be an unpleasant illness and can cause serious complications. Vaccinating your child will also help protect more vulnerable family and friends by preventing the spread of flu.
Please complete the consent form by following the link below (one for each child):

Your child’s school will let you know the date when the vaccinations will take place. Once your child has received their flu vaccination, you will receive an email to inform you of this and a list of the possible side-effects.
‘The nasal spray is the preferred flu vaccine for children. It is quick, painless, easy to administer and may be better at reducing the spread of flu to others than an injected vaccine. There will be a small number of children in the school not able to have the nasal vaccine because of pre-existing medical conditions or treatments. They will be offered an injected vaccine. The injected vaccine is also available for children whose parents don’t want them to have the nasal spray because of the very small amount of porcine gelatine (used as a stabiliser)’ (NHS 2024).
